Sometimes you need to access your full medical records. Patients may choose to have their medical records transferred from their referring doctor to UT Health Austin or, they may wish to send their UT Health Austin medical record to another doctor. Releasing Your Medical Records
For a copy to be sent to another party, please complete a Release of Information Form. A patient or his/her legal representative may request a copy of a medical record. Completion of this form allows UT Health Austin to transfer the medical record in compliance with the requirements for protection of patient health care information (HIPAA). Patients may obtain a copy of the authorization for the use and disclosure of protected health information form by:
- Request a Release of Information Form from the UT Health Austin office where you received medical care so we may request the records on your behalf.
- Authorization To Receive Records Form - to allow UTHA to request your medical forms from another provider
- Call UT Health Austin Access and Outcomes Center at 1-833-UT-CARES
- Complete the Release of Information Form at home and FAX to 512-495-5680.
Requesting Your Medical Records
A patient may request their medical records from an outside provider (like your referring doctor) to be sent to a UT Health Austin provider. In order to give the outside provider permission to send your medical records to UT Health Austin please complete the Authorization to Receive Records Form. Once you’ve completed the form, please give it to your current provider so they may send your medical records to us. MyUTHA is your secure, online connection to portions of your UT Health Austin electronic medical record. Through your personal MyUTHA portal account you can see test results, access information about the health issues that are of interest to you, and request new appointments.
