Medical Records

There may be times when you need to release or request your full medical records. Patients may wish to release their UT Health Austin medical records to another provider or they may need to request their medical records from another provider be sent to UT Health Austin. In either case, we are here to help. To initiate this process, please follow the instructions below.

To request a copy of your UT Health Austin medical record be sent to another party, please print, complete, and fax a Release of Information Form (also available in Spanish) to 1-512-495-5680.

A patient or their legal representative can complete the Release of Information Form. Completion of this form allows UT Health Austin to transfer the medical record in compliance with the requirements for protection of patient healthcare information (HIPAA). To request a copy of your medical record from an outside provider, such as a referring doctor, be sent to UT Health Austin, please complete the Authorization to Receive Records Form and (also available in Spanish) give it to your current provider so that they can send your medical records to UT Health Austin.
