Please note our updated masking guidelines as of May 2023 in the FAQ below.

You will receive an email or text message from our Access Center, which will include a link to your pre-appointment questionnaire. If you do not have electronic access, make sure to arrive at least 30 minutes early to your appointment to fill out your questionnaire before you are seen. The questionnaire will include questions about your health, your health history, and your care goals. Some of these questions may be sensitive, but they help your care team determine the right care plan for you.*

Benefits of completing the questionnaires at home:

  • Taking your time and saving time at check-in; the questionnaire takes an average of 10-15 minutes to complete
  • Completing sensitive information in a private, comfortable space
  • Allows your care team to create an individualized care plan, before your first visit, to help you reach your goals faster

We recommend that you register for the UT Health Austin patient portal here to view your patient summary, lab and imaging results, and to message your care team.

We are happy to announce that self-scheduling is now available for follow-up appointments for established patients. An established patient is an individual who has been seen previously by a provider in one of the specialties below and has an established MyUTHA Patient Portal account. Self-scheduling may be completed as early as 72 hours before the appointment date and time you wish to select provided availability. Check the appointments section on each page below to learn what types of appointments can be scheduled.

New patients, except for the Walk-In Clinic, should call 1-833-882-2737 to register, set up their portal account and schedule their first appointment.

Clinics offering self-scheduling:

* The Walk-in Clinic will accept new patient appointments for same-day, next-day care and within 2 hours of an available time.

Please arrive at least 30 minutes before your appointment. If you have not completed your pre-appointment questionnaire (see “What happens before my appointment?” above), you will be asked to do so at check-in. You can complete the questionnaire privately or our trained concierge staff (they will be dressed in teal shirts) can help answer them with you.

Please bring the following to your appointment:

  • Identification, such as Driver’s License or State-issued ID, Social Security Card, Passport, Birth Certificate, Military ID Card, or Employment Authorization
  • Insurance card(s)
  • Payment via MasterCard, Visa, American Express, Discover, cash, or personal check
  • Complete list of medications you are currently taking, including any over-the-counter medications, vitamins, or supplements
  • Complete list of other physicians you see, their contact information, and why you see them
  • Results from any other tests performed since your last visit, including those ordered by physicians outside UTHA

Given the continued decline in community transmission rates, UT Health Austin no longer recommends universal masking for our patients and providers. We will continue to provide and support masks for those whose who are under our care at this facility and whose conditions put them at higher risk for disease severity. Additionally, if you have a runny nose, fever, cough, or sneezing, we ask that you wear a mask while in this facility.

Should community transmission rates of specific pathogens increase, universal masking may be recommended in the future. We will continue to monitor the situation closely. We appreciate you caring for our patients as we care for you.
