Since March 3, 2020, when you call or visit UT Health Austin, you will be asked a set of questions as part of our COVID-19 precaution measures to protect patients and staff.

You will receive an email or text message from the Access and Outcomes Center, which will include a link to your pre-appointment questionnaire. If you do not have electronic access, make sure to arrive at least 30 minutes early to your appointment to fill out your questionnaire before you are seen. The questionnaire will include questions about your health, your health history, and your care goals. Some of these questions may be sensitive, but they help your care team determine the right care plan for you.*

*Beginning March 3, 2020, when you call or visit UT Health Austin, you will be asked a set of questions as part of our COVID-19 precaution measures to protect patients and staff.

Benefits of completing the questionnaires at home:

  • Taking your time and saving time at check-in; the questionnaire takes an average of 10-15 minutes to complete
  • Completing sensitive information in a private, comfortable space
  • Allows your care team to create an individualized care plan, before your first visit, to help you reach your goals faster

Please arrive at least 30 minutes before your appointment. If you have not completed your pre-appointment questionnaire (see “What happens before my appointment?” above), you will be asked to do so at check-in. You can complete the questionnaire privately or our trained concierge staff (they will be dressed in teal shirts) can help answer them with you.

Please bring the following to your appointment:

  • Identification, such as Driver’s License or State-issued ID, Social Security Card, Passport, Birth Certificate, Military ID Card, or Employment Authorization

  • Payment via MasterCard, Visa, American Express, Discover, cash, or personal check
