Please read how UT Health Austin is protecting patients and healthcare workers against COVID-19 here.

Where is the Health Transformation Building (HTB)?

The Health Transformation Building (HTB) is located at 1601 Trinity St., Bldg, A on the south side of The University of Texas at Austin campus. Parking for the HTB is available in the attached Health Center Garage (HCG). Download our map (PDF) or get personalized directions here.

What happens before my appointment?

You will receive an email or text message from the Access and Outcomes Center, which will include a link to your pre-appointment questionnaire. If you do not have electronic access, you may complete the questionnaire before your appointment, just make sure to arrive at least 30 minutes early. The questionnaire will include questions about your health, your health history, and your care goals. Some of these questions may be sensitive, but they help your Care Team determine the right care plan for you.

Beginning March 3, 2020, when you call or visit UT Health Austin, you will be asked a set of questions as part of our coronavirus precaution measures to protect patients and staff.

Benefits of completing the questionnaires at home:

  • Taking your time and saving time at check-in; the questionnaire takes an average of 10-15 minutes to complete
  • Completing sensitive information in a private, comfortable space
  • Allows your Care Team to create an individualized care plan, before your first visit, to help you reach your goals faster
